Warranty Request Form

Thank you for your purchase of a CODEL Door. There are two warranty forms below. Please fill out the appropriate warranty form if you are a Homeowner; or if you are an authorized CODEL Dealer filling out the warranty form on behalf of a homeowner. There are several pieces of information needed (photos of the issue, the CODEL order number, product information, purchase information). Please Note: Be sure to provide all of the required information requested, as any missing supporting documentation or photos will delay the process of the claim. We will contact you after we review the claim information.
